Fix local index.html demo
...
When serving statically, `process.env.REACT_APP_STAGE` is never set,
nor was `.export-links` found on the page, so the chart elements were
never being added.

* Renamed App to DatasetteVega
* Component uses fetch() to retrieve columns on start
